Etobicoke clinic uses $4.3M to attach 4,370 patients under provincial primary care plan

What’s happening

• The province launched the Primary Care Action Plan (Jan 2025) to connect everyone in Ontario to a family doctor or primary care team by 2029.
• Rexdale Community Health Centre in Etobicoke received $4.3 million, began attaching patients in August 2025, and has connected 4,370+ patients (42% of its 10,475 target).

By the numbers

• Provincial investment: $3.4 billion to support the plan
• 199 teams funded in earlier rounds to connect 800,000 people
• 124 successful applicants in the latest call expected to connect another 500,000 people
• Goal: connect roughly 2 million more people to primary care by 2029

Why it matters

• Primary care teams (doctors, nurse practitioners, nurses, physiotherapists, social workers, dietitians) help people get routine care closer to home and can reduce pressure on emergency departments.
• The plan includes a provincewide Primary Care Medical Record to better share patient records and reduce clinician paperwork.
