Brantford clinic signs up nearly 3,200 patients with provincial funding

What happened

• Grand River Community Health Centre in Brantford received $812,600 from Ontario’s Primary Care Action Plan and began attaching patients in August 2025.
• The team has signed up nearly 3,200 patients, reaching about 121% of its original target of 2,625.
